Job Description

  • Ensure the provision of customer focused care and safety in the med-surg setting.  Carry out responsibilites within the framework of the established standards of patient care in nursing practice. Participates in direct patient care at least 25% of the time.
  • Must comply with all applicable policies and procedures.
  • Must initiate assessment data and identify specific patient physical, psychological, social, education, safety and discharge planning problems/needs by utilizing CCM's when disease appropriate.  Problems and needs will be addressed in the patients plan of care.
  • Must assure that a plan of care is written for each assigned patient and checks it daily appropriateness.
  • Must identify the need for further patient assessment and refer patients to other professional services in the organization to ensure that the need for further reassessment is met.
  • Plans with other hospital areas for the coordination of clinical interdepartmental activities based on patient needs.
  • Must modify plan of care based on reassessment of the patient including observed patient response to treatment/care and evaluation of patient's progress toward goal completion.
  • Must advise physicians, other disciplines, family/significant others of changes in patient's condition or abnormal findings.
  • Must observe, document, address, and implement planned interventions/care and then communicate the plan to all members of the care team.
  • Must ensure that specific educational and discharge needs identified in the assessment process are met.
  • Implements the medical plan of care based on physician orders.  Must assist physicians with procedures and treatments when needed.
  • Must ensure the Standards of Care, Standards of Practice, Standards of Competency, Nurse Practice Act, TEAMSTEPPS, Standards of Excellence and nursing policies and procedures are followed by the team member on the nursing unit.
  • Must administer medications, know their actions and side effects, recognize allergic reactions and document the patient's response to drugs.
  • Must answer call lights.
  • Precepts new employees and oversees other members of the care team.
  • As a member of the care team, the Care Coordinator will conduct themselves in such a manner that demonstrates respect for the individual dignity of patients, families, fellow associates and physicians and fulfills the mission and values of Riverview Hospital.
  • The Care Coordinator supports and communicates the mission and values of Riverview Hospital to their team.
  • Must demonstrate a strong clinical knowledge base and use it in the assessment and care of patients.
  • Must be responsible and accountable for taking the initiative to perform tasks.  They must recognize and adverse patient response and initiate appropriate interventions.
  • Must provide professional nursing care.
  • Performs selective procedures that include but are not limited to IV starts and restarts, blood administration and phlebotomy tasks.
  • Must coordinate care/interventions with other members of the care team by delegating appropriate tasks.
  • Must promote a sense of team spirit by creating a culture of mutual respect and high performance expectations among all team members.
  • Must provide clean and direct communication.
  • Must coach and guide the team to continuously seek ways to improve patient care and to improve as a team.
  • Must address and facilitate the resolution of conficts arising within the team and between the team and other care providers, utilizing negotiation, consensus building and constructive criticism.
  • Must refer issues requiring formal corrective action of team members to the Unit Manager and/or the Clinical Director.
  • The Care Coordinator may particiapte with the Unit Manager and the Clinical Director in the selection of candidates and yearly evaluations of team members.
  • Must actively work to ensure that patient rights are identified and met for every patient.
  • Must provide care based on philosophical and ethical concepts.
  • These concepts include reverence for life; respect for the inherent dignity, worth, autonomy, and individuality of each human being; and acknowledgeing the beliefs of other people.
  • Must be knowledgeable of and demonstrate skills in the principles of infection prevention, reduction and control.
  • Must complete documentation for restraints if patient appropriate.
  • Must make walking rounds and give report to oncoming shift.
  • Must be able to safely and effectively use medical equipment in carrying our planned interventions.
  • Must remain current and competent in scientific knowledge and clinical skills necessary to meet the responsibilities of this position.
  • If applicable, must function as testing personnel under CLIA1988 regulations as they relate to point of care testing.
  • Must attend mandatory inservices and other educational offerings as encouraged by the Clinical Director.
  • Must obtain competencies for performing new techniques or skills withone year of being in the position.

Job Requirements

  • Level of Education
    • Minimum: Graduate from an accredited school of nursing
    • Preferred: BSN
  • Level of Experience
    • Minimum: At least one year of RN experience
    • Preferred: Certification in Med-Surg, Orthopedics or Chemotherapy
  • State Issued License - Yes
  • CPR - Yes
  • ACLS - Yes within one year of hire.
  • PALS - N/A
  • Specialty - Telemetry within one year of hire.
