POSITION SUMMARY
- Is responsible for development and coordination of inpatient and outpatient diabetes education. Includes both patient and family/significant others whenever possible. Acts as a resource person for hospital staff, medical staff and the community. In compliance with HIPAA minimum necessary review, this position requires unlimited access to the medical record or PHI access for patient care activities.
- Must coordinate the education of hospitalized diabetic patients and significant others.
- Must collaborate with nursing staff in the daily identification of patients who need instruction and follow-up education.
- Must assess learning needs of each patient prior to establishing a written plan of care.
- Must establish an individualized teaching plan with stated educational objectives for each patient.
- Must act as a resource person for patient/significant other following discharge.
- Must encourage staff participation in the teaching of the patient.
- Must attend unit patient care conferences as needed.
- Must be available to assist Social Service for coordination of patient’s home care needs.
- Must consult with physician regarding patient’s nursing and educational needs.
- Must develop and maintain a method of tracking diabetic patient data.
- Must provide instruction regarding SBGM (self blood glucose monitoring), medication, exercise and general diabetes care to patients, families, and nursing staff.
- Must provide written and verbal discharge instructions concerning treatment regimen at home.
- Must develop the Diabetes Outpatient Program to include groups and individual classes. Must coordinate and direct multidisciplinary team approach to outpatient diabetes care.
- Must work with healthcare team in providing a plan of care for each outpatient.
- Must assess patient learning needs and provide individual instruction when necessary for outpatients.
- Must consult with physician regarding the outpatient treatment plan.
- Must provide both written and verbal communication to physicians regarding referral process for education classes.
- Must work with Community Relations to promote diabetes education classes to hospital staff, medical staff, and the community. Must formulate, review, document, and develop educational materials and patient record forms as needed.
- Must participate in the revision and review of diabetes related nursing policy and procedures and submit to NPP Committee for approval.
- Must act as a liaison with pharmaceutical and supply representatives to facilitate exchange of information of current products and literature available.
- Must participate in staff development workshops and new employee orientation when needed.
- Must collaborate with the Education Coordinator in assessing the learning needs of the nursing staff.
- Must comply with all applicable policies and procedures, including, but not limited to: all departmental and functional policy and procedure, maintaining acceptable attendance records, arriving for duty punctually, and following dress code.
- Must participate in performance improvement activities as requested.
- Must assume information management responsibilities.
- Must have knowledge and skills pertinent to the functional approach to patient care and the organization.
- Must perform duties as an active member of the healthcare team.
- Must demonstrate a knowledge of team dynamics and contribute to team building and skills related to participation in and strengthening of the multidisciplinary team approach to the delivery of patient care and organization function. Normally works from 8:00 a.m. - 4:30 p.m..
- Must be able to work overtime.
- Must be able to work both weekdays and weekends as well as holidays as requested.
- Other duties as assigned.
Job Requirements:
EDUCATION AND EXPERIENCE

Level of Education
- Minimum: Health Care Professional who is a Certified Diabetes Educator

Level of Experience
- Minimum: 3 years experience in clinical nursing / dietitian practice
Must have acquired specialized knowledge related to the care of the diabetic patient.

- Type of Specialty Certification or other Licensure, if required: - Registered Nurse, Registered Dietitian, Pharmacist or Physician who is a CDE or has achieved Board Certification in Advanced Diabetes Management (BCADM)
